Prior to the beginning of School, a physical examination, including a hearing, vision and spinal screening, must be completed or transferred for each new student entering the School. Immunization record or exemption certificate from the state is required for all students. Immunizations must be kept current, and an immunization record or an exemption certification must be kept on file in the School office. Students may not attend School without an appropriate immunization record.
For returning students who receive an immunization on their birthdays, please provide the Health Office with the appropriate form denoting the type of immunization and date received.
In addition to immunizations, children 4 and 5 years of age or either in 1st, 3rd, 5th , 7th or 9th levels are to be screened for vision and hearing. The results of this screening are to be provided to the Health Office at the beginning of the school year.
No child will be admitted to class if all State health requirements are not met, including an updated immunization record or notarized form from the State indicating exemption from immunization. The required immunizations are contained in the Texas Administrative Code, Title 25, Health Services, Part 1, Chapter 97, Subchapter B, Section 97.63, a copy of which is on file in the front office.
Please note: Each parent or guardian who signs a vaccine exemption affidavit form is acknowledging they understand that their child will not be allowed to attend school in times of an emergency or an epidemic declared by the Texas Commissioner of Health.
